本計劃提供眾多網絡醫生以供選擇，遍佈港九新界，方便周全，而各種專科醫療服務定能照顧您不同之需要。您只需在求診時出示此醫療咭，並繳付固定收費即可，毋須任何額外手續，簡單快捷。

條款及細則﹕

• 各項保障只限每天接受診症一次。

• 閣下須備有效之普通科醫生轉介信方可接受專科醫生診症。

• 網絡醫生保留權利要求會員出示身份證明以核對身份。

• 如 閣下需要特別之醫療服務 (如包紮、注射等)，又或所需之藥物乃性質特別或費用昂貴，網絡醫生保留權利按個別情況收取額外費用。

"Health Concepts' Carecard" brings you quality and reliable general and specialist outpatient services. At just HK$120 per year, you can become a member to enjoy discounted medical consultation services, and extra 5% discount on Preventive Cancer Screenings.

Our panel doctors are located in convenient and accessible locations throughout HK, Kowloon and the New Territories. Accompanied with our wide variety of specialist services, we must be able to satisfy the different needs of you. Simply present the Carecard and make your fixed amount of payment whenever you visit a doctor.

Terms & Conditions:

• Each coverage is limited to one single consultation per day.

• Valid referral letter should be required for specialist’s consultation.

• Member may be requested to present his/her ID card for verification.

• Panel Doctor reserves the right for extra charges on dressings, injections or medications if the prescription required is special and excessively expensive.
